This is a really important point. Nobody is saying come off the diet. I myself have been low-carb/keto since 2018. What most of us are advocating is that for those people who are on these diets and find benefit and also have these increases in LDLc/ApoB, do not assume that it is safe from the perspective of ASCVD risk. Instead, most would advocate to either 1) make some modest changes to diet (add fiber, reduce SFA, slightly add back some carbs), or 2) add a medicine to reduce ApoB (ezetimibe, statins, PCKS9i or other).
Importantly, this does not mean that there is zero chance these increases in ApoB are safe(er) in the context of a low-carb diet. What it means is that given the extraordinary amount of evidence linking ApoB to ASCVD risk in all other contexts, the default assumption should be that this is no different in this context and that until and unless it is shown categorically in at least one well-powered clinical study looking at hard clinical outcomes, nobody should assume that this context is any different from any other context we have seen yet

Agree that it’s great to see this. Delivering 2.3 will be key. I don’t doubt the funders desire to see this happen. I worry about the reality of commitment from NHS. Colleges and particularly HEE, which seems to be intent on stifling excellence and academic aspiration
