If a brand med has a $1k retail price, if there is any competition, it has as much as an 80% rebate going to the PBM. The PBM may pass a chunk of that to a plan sponsor. They don't for uninsured or High Deductible plans. They also don't reduce the cost for those with co-insurance plans
The patient pays $1k or $200 for their coinsurance.
Unfortunately, those biggest PBMs scare big pharma to not sell to @costplusdrugs and pass through PBMs we partner with. They say without saying that they will remove or deemphasize their meds from formularies. Costing them billions
If we could get rebates, even smaller, from them, with say a 70% rebate, our price for that $1k med would be $345. The coinsurance would be $69
Maybe not as low as global prices on all cases. But better in some.
That's the first step. That's the easiest step.
If we can convince self insured companies to leave Big 3 PBMs and use pass through PBMs, that will force big pharma's hand and they will need to work with us.
The best part is those CEOs will live up to their fiduciary responsibility and save their companies money and improve their employees wellness.
Ask @elonmusk and @howardlutnick and @pmarca and other CEOs you are working with to move their benefits contracts away from the big PBMs and Big Insurers and the healthcare business changes over night
