Beta blockers are not that helpful in heart failure. Only patients with mildly reduced heart function—specifically those with a left ventricular ejection fraction of 40 to 49 percent—may still derive benefit from beta-blocker therapy.
The REBOOT-CNIC trial, led by Borja Ibáñez and a large international team of co-investigators, was published in the New England Journal of Medicine in 2025. This large open-label randomized controlled trial enrolled over 8,500 patients from Spain and Italy who had been invasively treated for a recent myocardial infarction (heart attack) and had a preserved or mildly reduced left ventricular ejection fraction (above 40%), meaning they did not have significant heart failure or reduced pumping function.
The study evaluated whether routine long-term beta-blocker therapy provided any benefit compared to stopping or not using beta-blockers in this population. Patients were randomized after stabilization, with the primary endpoint being a composite of death from any cause, new myocardial infarction, or hospitalization for heart failure. Results showed no significant difference in the primary outcome between the beta-blocker group and the no-beta-blocker group, indicating that routine beta-blocker use did not improve clinical outcomes in these patients.
These findings challenge longstanding clinical practices that originated in an era before modern reperfusion therapies, potent antiplatelet drugs, and statins were standard. The results were largely consistent across subgroups, suggesting that beta-blockers may not be routinely necessary long-term after myocardial infarction for patients with preserved ejection fraction and no other compelling indications. This could help simplify post-MI medication regimens and reduce potential side effects for many patients.
