In this article, @Obisht, Won-Keun Kim and Robert R. Flieger describe a self-made steerable catheter system using a 6F EBU guide and the stiff back end of a 0.035″ wire, combined with a hydrophilic straight-end crossing wire, to facilitate crossing of severely stenotic or calcified native or prosthetic aortic valves during TAVI.
The technique is particularly useful in horizontal aortas, bicuspid valves, and massively calcified tricuspid or bioprosthetic valves, where the conventional technique repeatedly fails.

Yeah this is bullshit. I have referred 3 Pts for Ross in the last 1-2mos. Anyone under 65 gets a big discussion from me, and anyone <55 I recommend mAVR or Ross primarily. This leaves aside the fact that there are a lot of unknowns about bAVR vs TAVR durability on index intervention vs 2nd/3rd intervention. Where is the discussion of CT surgeons recommending CABG when the SYNTAX in <32 and there is zero mortality benefit? Do surgeons consent patients and tell them there’s a 50% chance their SVGs will be included at 10yrs and 20% chance of SVG occlusion at 1 yr? Never. This conversation always biases against the proceduralist and never against the surgeon.

Criticism of STEMI-DTU is revisionist. Biologically grounded strategy supported by years of preclinical work. We cannot claim to value innovation, then ridicule a rigorously tested hypothesis because the trial was neutral. Hypocrisy deserves to be called out. @NavinKapur4
CHAMPION-AF: My thoughts, especially in the context of CLOSURE-AF.
A quick summary of the trial and main findings:
1. 3000 patients randomized in 1:1 DOAC vs Watchman Fix, mean age 71, avg CV2 3.5.
2. Primary efficacy endpoint composite of CVD, stroke/SE

Study conclusion: Angiography guided PCI was associated with significantly higher risk for MACE, However, IVI-guided PCI had comparable risk with CABG in DM patients.
But there's a methodological concern hiding in the study design that the authors didn't discuss:
Immortal time bias.

Anyone who worked as an interventional cardiologist before the introduction of Stents , knows that the current direction to promote DCB as stent free PCI world will fail.
What we really need to know is if we can use DCB when and where we do not want to put DES,like distal LAD.
