There’s a substantial number of gov / mil programs out there that consistently fail to produce results due to the participant’s own moral standards. When you’re assigned with such inhumane or unjust tasks, a lot of little ‘errors’ will compound to force a failure while appearing good on paper. This goes well beyond CBRN-related warfare. This reminds me of Cold War incidents where they decide not to launch nuclear missiles despite that being the protocol.

We have a tremendously vulnerable national infrastructure and a serious lack of will to secure it before we pay in compromises… Unfortunately the industry standard of “wait and see” will likely hold.
