Olmesartan induced enteropathy represents the primary pharmacological link, where this specific ARB mimics celiac disease to cause severe, sprue like chronic diarrhea.
Dehydration and severe fluid loss from chronic diarrhea can trigger acute kidney injury (AKI) in hypertensive patients, particularly when combined with diuretics, ACE inhibitors, or ARBs.
Shared systemic etiologies like neuroendocrine tumors (e.g., VIPomas or carcinoid syndrome) can simultaneously manifest with both blood pressure dysregulation and profound watery diarrhea.
Clinical drug withdrawal remains the definitive diagnostic and therapeutic strategy if medication induced enteropathy is suspected as the underlying trigger.
I was reading everything related to the Ankita Bhandari murder case.
She had said, "I may be poor, but I won’t sell myself for ₹10,000 to your resort."
She had started working there only 3 weeks back.
She must get justice.
“5 CBC mistakes residents make” 5/5
🩸 CBC Mistake #5: Pancytopenia = leukemia
Pancytopenia is a pattern, not a diagnosis.
Think broadly:
🔹 Drugs/toxins
🔹 Nutritional deficiency
🔹 Viral infections
🔹 Hypersplenism
🔹 Aplastic processes
🔹 MDS
🔹 Acute leukemia
🔹 Marrow infiltration
Then look at the reticulocyte count + peripheral smear.
📌 Low reticulocytes → production problem until proven otherwise.
📌 Abnormal cells on smear → stop and investigate further.
The CBC is the beginning of the hematology work-up—not the end.

Urine cell-free RNA profiling (uRARE-seq) detected bladder cancer with 95.7% sensitivity versus 34.4% for urine cytology in 93 paired samples - including 90% sensitivity in low-grade Ta (non-invasive) tumours.
📍 Losartan - only ARB with uricosuric effect - inhibits URAT1 transporter in proximal tubule - blocks urate reabsorption - increased uric acid excretion - lowers serum uric acid; this is a unique molecular property of losartan itself, not a class effect - other ARBs (valsartan, telmisartan, irbesartan) lack this uricosuric action
📍 Clinical relevance - preferred antihypertensive in hypertensive patients with gout/hyperuricaemia - treats blood pressure + simultaneously lowers urate; particularly valuable in patients on thiazides/loop diuretics (which raise uric acid) - losartan partially offsets diuretic-induced hyperuricaemia; the uricosuric effect resides in losartan itself, not its active metabolite E-3174
Happy to share a special milestone from Bengaluru.
This child was diagnosed with #SMA type 1 at just 1 month of age when visited to my clinic with weakness of all 4 limbs. following which we initiated risdiplam without delay. In parallel, we began the challenging process of raising funds through crowdfunding to make gene therapy possible.
After months of coordinated effort involving the family, donors and our multidisciplinary team, we were able to successfully administer #onasemnogeneabeparvovec (Zolgensma).
For me, this has been a particularly fulfilling journey—from early diagnosis and initiation of disease-modifying therapy to mobilizing resources and finally delivering gene therapy.
Sharing a picture of our team from the day of infusion.

90 days of discipline can fix the mess of the last 9 months.
So don’t lose hope.
Revise. Solve PYQs. Stay away from distractions.
Your rank can change completely in the next 90 days.
NEET PG is yours. 🔥
Dr Arthur Guyton had 10 children including a professor of ophthalmology, a professor of surgery, a professor of medicine, a cardiothoracic surgeon, a rheumatologist, two anaesthesiologists and two orthopaedic surgeons. What a man.
Big fan of @hyderabaddoctor sir
A cerebral venous sinus thrombosis in a 59 year old man
Likely provoked by dehydration
But thrombophilia screen showed a homozygous factor V Leiden mutation
I really appreciate his skills here
I know plenty of people who would never have sent a thrombophilia screen in this patient
In fact his resident opined the same
But it provides a diagnosis and changes treatment
This patient will need lifelong anticoagulation
Mammalian biology is not so fragile that mere dehydration will cause a clot to form in the venous system
Always look for a proximate cause
Rare diseases = not rare
They are rarely diagnosed
Thank you sir for this teaching case!
