How could someone end up on 18 Ψ meds?

1. Came for anxiety -> Zoloft
2. Mentioned public speaking anxiety -> Propranolol
3 & 4. Got anxiety on Zoloft -> Valium + prn Xanax
5. Got sleepy on Xanax -> Adderall
6. Got paranoid on Adderall -> Risperdal
7. Mentioned nightmare once -> Prazosin
8. Got EPS on Risperdal -> Cogentin
At this point, symptoms are more nebulous as the patient is over-medicated
9. Still depressed -> Effexor
10. Continued insomnia -> Trazodone
11 & 12. Continued anxiety -> Vistaril, Buspar
13 & 14. Unstable mood -> Lamictal, Lithium
15 & 16. Continue insomnia -> Seroquel, Melatonin
17. Continue sleepiness during the day -> Modafinil
18. Losing weight -> Remeron

To be clear, I have never seen this particular case. This was more of a thought exercise. I commonly see 6-8 Ψ meds, occasionally a few more.
