Let's be explicit: "nonprofit" is a TAX STATUS, not a mission statement.

The majority of hospitals in the US are "nonprofit", but "nonprofit" doesn't mean not $ motivated. 🧵

Here’s how the model works: nonprofit status means tax cuts. Huge tax cuts. On the order of $28 billion/year for the nonprofit hospital industry.

In exchange, they are expected to give a certain amount of “charity care” (aka care for the uninsured). As an industry, this ends up being much less charity care than their tax benefit — a $12 billion difference (“ahem, profit”) to be exact.

In addition to the $28b in tax benefit, they can also get “charity care” payments (disproportionate share payments (DSH)) from the government...$31 billion/year of additional payments.
