After explaining that I can only send it to the email I'm told to, I was then given another email which I again sent the evidence to and was told I'd have a response the next day.

I chased 3 times then on 19th Sep, was told it wasn't being looked into and was given an email address to send evidence to so that it could be. I did. I chased 7 more times. Then on 30th Oct I was told I'd sent it to the wrong email (the correct one as far as I was told).

He's basically told he's going to get the ball 4 times and one of them he needs to either assist or score a goal

Maybe when a physio comes on, that player is off the pitch for 2 mins ?
If you're that hurt you need medical assistance then it only makes sense you can't instantly return to play.
