@jasonryanmd It is the system that contributes to the error. Over and over, the institution points to an individual to shift blame but it is always a system failure.
@georgetolisjr As a scrub, I had to have pledgets of multiple sizes ready. I had to make sure I could read the sizing before I handed it to the surgeon. I know someone forgot to check the orientation before handing it to the fellow. It is a system failure.
@DrJesseMorse I know we should focus on the system. Which OR RN didn’t know that you must properly orient the sizer before you pass a stitch? It really does come down to properly trained OR staff!!!
@DrJesseMorse Maybe this is not time for you to speak. This has always been made hard to do wrong, easy to do right. I am the scrub for Drinkwater, Jeevanandam, McCarthy, and Karla Christian too. Do not try and justify this egregious error. It happened. To err is human.
So much noise about an MD whose patient had a bad outcome. The only way for surgeons to never have a bad outcome is to never do a surgery. Mistakes happen and the solution is the system needs to be failsafe. Look at the system, not the surgeon.
