in other news, began on rehab ward (off-campus) today. pt had code blue (GCS 3) and ph 7.28 & lactate 3.7 (vbg), CRT~5s, oliguria (70mL at 8hrs). registrar decided pt is fine & stay at subacute (no doctors onsite till wednes, no onsite pathology/imaging). graded escalation my ass

dude was a bit of a mystery, at one point on 3x IV abx, myeloma/haemolysis screens both neg, USS unremarkable, just ongoing fecers, persistent pancytopenia w/ proper neutropenia & plts of like 25, sodium tanked, mg wouldnt stay up, bilirubin climbing to 30s...wild times!

okay so answer seems to inc: thal + hypersplenism + ineffective erythropoiesis + transfusional Fe overload + MDS w/ atrx mutation!

recently saw very similar picture - septic pt w/ acute decomp CLD (CPB,etoh excess). Rx: abx (SBP vs febrile neutrop)+pRBC +LOTS of conc alb (resus+hypoNa) +electrolyte mx. fbe/uec/lft improved but hypoMg hard to fix (cant recall why), planned repeat Fe studies before infusion
