Tendon pain is the most common reason people come to see me.
Most of it is self-inflicted — from doing too much, too soon, or from doing too little for too long.
Let's review what most people (including many doctors) don't understand about why tendons hurt and how to fix them. 🧵
Nearly 30 years in orthopedic surgery. Another pattern that is impossible to ignore.
Perimenopausal women are some of the most undertreated patients I see. One unfortunate paper is blamed... but often, it's because the conversation never happens.
@hjluks i’m not familiar with this book by Codman but what I have seen of the literature doesn’t even necessarily support the freezing/thawing stage completely nor does it give a definitive stance on MUA outcomes? Thoughts on this interaction i saw on IG?
@hjluks@ProfJillCook I have trouble working with stubborn tendon cases the most. How much pain is acceptable? How do we differentiate whether we are providing TOO much stimulus or stress to the tendon and worsening the situation? How much load is “heavy”? I have a lot to learn about tendon mgmt
@hjluks Dr. Howard, Israel now has control over all borders and is seemingly prepared for a large scale assault on Rafah while it contains over 1.3 palestinians, a majority being innocent civilians. Do you truly not see how disproportionate Israel’s actions are?
@hjluks You cannot make any argument that this is Israel exercising their right to defend themselves. This goes beyond any of that, it is unconscionable.
@hjluks Your claim about the ratios are highly controversial at the moment considering data tracking on casualties are limited, but most independent organizations are claiming almost 15000 children killed. If you think this is excusable for the sake of war, i fear you are too far gone.
Cigna denied a reverse total shoulder. The reason? "Good deltoid function" was documented.
For those unfamiliar, good deltoid function is a requirement for a reverse total shoulder.
It would be laughable if there wasn't a person being harmed by these greedy insurance companies.
@hjluks Thank you! I was a big advocate for starting exercises and movement as early as possible, especially if their pain levels were more manageable but my CI disagreed cautioned against so i figured a second opinion would be helpful
@hjluks Hi Dr. Howard, im a PT student recently training at a clinic. I used this thread to help guide my evaluations of FZsh a lot so thank you for that. My CI was worried about early resistance training causing flare ups in pain and accelerating swiftness. Do you disagree?
@Paddy_Barrett This was refreshingly simple and actionable. Of course the comments are still riddled with people complaining about insulin, carbs, etc. despite the body of research we have now regarding losing weight. But still, i was pleasantly surprised
