Agencies and individuals want a piece of the pie. They use the government to acquire "their" share. Costs have to be absorbed somewhere. Cost and overhead is first day of any business class. Its also simple logic that costs are passed on to the consumer of those services or goods. They don't just eat it, UNLESS the government forces them to. And that's the situation you have between insurance companies/government and Hospitals and Doctors. Therefore, costs rise and so does the fee to the patient.
