@Methadone_Cat I think there’s two issues that kind of get muddled
One being how bad the BUP/naloxone formulation can be for some people, both at killing pain and at reducing cravings. Also there are the awful side effects of long-term naloxone absorption that no MDs seem to care about
Buprenorphine isn't specifically a painkiller. For a lot of patients, most in my experience, it does very little to control pain. You need a full agonist for proper pain control, especially when dealing with acute illness or injury that would traditionally call for morphine.

But regardless, sound & relevant research that isn’t slanted against LTOT is super important
People with intractable pain are more likely to need LTOT. Things like CBT are NOT going to work and haven't worked for these people and most of them have already tried everything else. Why are we spending billions on research without acknowledging this monumental difference? 🧵
