The cause of cancer was understood in the late 1800's. At that time, real scientists would obtain cancer specimens, meticulously study them, observe the specimens under a microscope for days on end and hand draw what they observed. Fungal spores, coccidia (intracellular parasites), etc. They understood how tumors grew.

Below are screenshots of a document from 1895 called "Micro-organism of Cancer". The focus of the document is fungus in its various forms and types.
