It would be nice if there was a way for these patients to be “traded” — assuming the patient wanted to do so — so that they could actually get care in the area that they now live in. A “trade network”.

Family medicine in Ontario needs a patient roster “trade network”. Hear me out.
If I have a patient who is rostered to me, but then moves across the province, it can be difficult to help manage their care.

I’m sure this scenario is going on all across the province. I probably have plenty of patients that live 2+ hours away. And I’m sure there are doctors in those cities that may have patients that have moved to where I practice.
In 2019, I was a CVICU patient in cardiogenic shock due to fam cardiomyopathy. Had to get a heart transplant!
This week, I’m a doctor in a CVICU. Got my Crit Care Medicine #fellowship diploma, & have a few shifts left. 🥹
