Check it out, important data for our brain metastases patients. We get asked a lot following WBRT or SRS, is there a chance for cognitive recovery, we found 38% have CR at 6 months and 68% long term recovery after initial decline. Pooled prospective data.

@StasLazarev At the end of the day, most pts will not likely benefit from it, but there does appear to a subset of pts that get meaningful benefit in quality and quantity of life
@StasLazarev Data for gliomas are the most challenging to interpret. Survival curves for RTOG 9802 did not separate until after 4 yrs. Most reasonable people in the field have interpreted the data as reflecting meaningful progress.
@StasLazarev "Seventeen patients received systemic therapy or convection enhanced delivery with omburtamab after initial RT". Need to be well enough to tolerated convection enhanced delivery and live long enough to get reRT (immortal time bias)
@StasLazarev I meant Temodar in GBM - the improvement in median survival not very impressive but the long term survival benefit is where it is most meaningful

@StasLazarev Best comparison drug is Temodar - most pts not likely to have huge benefit but significantly increases number of long term survivors. Next step is to identify pts most likely to benefit
@StasLazarev JCO paper excluded DIPG but earlier work from Koschmann group included pediatric DIPG pts showing median OS of ~22 months which compares very favorably to prior COG trials (ie PNOC003 had OS of ~13 months)

@AshwinShindeMD@UTSW_RadOnc@MHLinPhD usually we do mr sim with our ct sim for gbm, so instead of mr sim pt had unity sim and mask made while acquiring mr. agree treatment times longer, but most useful application is starting symptomatic pts quicker (essentially just a fancy sim and treat)
