Pls move NAC to top priority. Upfront cystectomy is much higher risk/resource drain than 3 visits to infusion cntr for ddmvac or 8 for GC. Risk of hospitalization w either is low. Chemo buys 6-12 weeks + 12 more as safe interval to surgery. Goal is cure.

Anyone else seeing this? 57 yo with low risk PCa referred for #RALP told him to do AS but we got an MRI to confirm low risk disease. Shows a 2.5 cm anterior mass PIRADs 5. offered repeat biopsy. Pt wants RALP. Aetna denies surgery. See letter. Thanks, but no thanks.
