Hmm. Some blame hospitals, others HMOs, and there are cases where approvals were reportedly given but patients still waited.

NHIA has made it clear emergency cases shouldn't be delayed and is taking action on reported breaches.

Both HMOs and enrollees need to do a better job of following the rules.
