I guess it was probably due to poor:
1) Fire safety management (production company)
2) Organisation + Communication (Newham Council)

Following a “planned” explosion during a filmshoot in Silvertown over the weekend – Newham Council will be working with our film office, and partners @LFBNewham & @RoyalGreenwich to understand what led to the incident. No one was injured.

This was pre-planned but film scheduling had been revised is my understanding.

Those responsible for the incident are usually in Hollywood - not Docklands!!

@LBCNews The council approved a film production request, however the dates appear to be slightly out. Although there is a BBC World News "truck" in some of the media surfacing, the BBC were not responsible or even involved in the incident! It's a future major blockbuster to blame!

Just read the report from the parliamentary inquiry into T1DE, chaired by @theresa_may and @georgehowarthmp. A very detailed report along with recommendations that not only need to be considered, they need to be funded and implemented. NOW!
