This is one of the many reasons why choice in our health care is so important. West Palm VA you need to do better than this. How is it 7 years later and many items are the same as before? The OIG identified findings similar to the 2019 OIG inspection, including inconsistent patient safety observation practices and inaccurate documentation, inappropriate assignment of additional staff duties, and facility policy that did not align with VHA requirements. Although leaders became aware that staff were not performing patient safety observations correctly and identified strategies for improvement, changes were not implemented. https://t.co/SXSY3jquRu
