1) Autopilot is not "Self-driving mode." Big difference.
2) Basically 100% of the time these stories come out it turns out to be the fault of the human driver. Tesla's vehicle logs will show the actual data whether Autopilot or FSD was turned on or not (it most likely wasn't).
