Adiala, Shifa and the Politics of Custody

Imran Khan’s reported return to Adiala Jail after a brief transfer to Shifa International Hospital should not be casually presented as proof that he refused medical care, accepted a deal, or was granted a favor. The facts still require disclosure: the medical findings, the doctors’ recommendations, the precise legal basis for his return, and whether Khan himself made any decision concerning admission.
Yet the political meaning of the episode is unmistakable. In Pakistan’s present climate, a hospital transfer is never only a medical matter. It can quickly become a carefully managed political spectacle. A prolonged stay at Shifa could have allowed the authorities to construct a narrative of benevolence: that the state, or General Asim Munir’s establishment, had acted compassionately to protect Khan’s health, dignity, and security. But medical care for a prisoner is not compassion. It is a legal, constitutional and moral duty of the state that holds him.
If Khan had any discretion in returning to Adiala, he may have calculated that he should not permit a court-ordered medical intervention to be converted into political credit for those responsible for his detention. A medical examination is not reconciliation. Treatment is not a concession. Hospitalization is not a back-channel settlement.
That distinction is especially vital where Khan, his family, lawyers, and political party face restrictions on access and communication. The custodial authorities possess the power to define the first public account: why he was moved, what treatment he received, whether he refused admission, and whether discussions occurred. In such a controlled environment, silence can be exploited and medical care can be repackaged as state magnanimity.
Still, principle must not become self-harm. Khan should never have to choose between needed treatment and protecting himself from propaganda. If inpatient care is medically indicated, it must be provided transparently: through an independent medical board, a written report for the Supreme Court, access for doctors trusted by his family, and prompt contact with counsel and relatives.
The central demand is straightforward: publish the medical facts. No propaganda, no implied bargain, no secrecy. A prisoner’s health is a right—not a favor to be traded for silence.

Former Prime Minister of Pakistan, Imran Khan is in solitary confinement since October 16, 2025.

He has spoken to his sons only twice in 2025 and once in February 2026 after Supreme Court orders. He has been denied books to read. He has received only 3 of the dozen books that we managed to get to adiala through a judge.

Then, a perfectly health and fit Imran Khan suddenly develops a clot in his eye. He was not provided medical assistance. For two weeks, he repeatedly told the jail superintendent that he CANNOT SEE.

The Adiala Jail superintendent Ghafoor Anjum DELAYED calling in a specialist for three months.

By the time the eye doctor came, the delay caused permanent damage to Imran Khan’s eye. We are extremely worried that his other eye could also develop a clot.

Imran khan has repeatedly sent messages over past few months that he is not satisfied with the treatment and there is only marginal improvement after the first injection in the eye. No improvement since then.

Over the past month, Bushra Bibi, his wife has also developed an eye condition and was operated upon last week.

Imran khan and his wife are being tortured through solitary confinement. We have repeatedly protested, demanding Imran Khan to be examined and treated at Shifa International Islamabad. There is an urgent need to identify the cause of the blood clot. Our worry is that his other eye could also develop a clot unless proper diagnosis is done and it’s treated accordingly

This is inhumane and a violation of all fundemental and human rights of a prisoner under Pakistan and International laws.

The hardest thing to explain to someone inside the imperial consensus is the concept of structural violence.

They understand individual violence.

One person harms another person.

There is a perpetrator and a victim and a clear causal chain.

What they cannot see, what the entire educational and media apparatus has been carefully designed to prevent them from seeing, is the violence that happens when a system is arranged so that certain people predictably die, predictably suffer, predictably lose, not because any individual decided to harm them specifically but because the overall arrangement of power requires their subordination.

The people of the Global South do not die of poverty because individual Americans wish them dead.

They die because the international economic architecture, the terms of trade, the debt structures, the conditions attached to IMF loans, the intellectual property regimes that prevent technology transfer, the agricultural subsidies that undercut developing world farmers, is arranged, in aggregate, in a way that concentrates wealth in already wealthy countries and extracts it from already poor ones.

And that architecture was designed.

It was negotiated.

It was implemented by specific people in specific rooms making specific decisions about who would benefit and who would not.

This is violence.

It does not look like violence because no one is pulling a trigger.

But the deaths it produces are just as dead.

And when you try to explain this to someone whose entire identity rests on the belief that what they have they earned, and what others lack they failed to achieve, you are not making a political argument.

You are dismantling the story that makes their life make sense.

They will not thank you for it.

They will defend against it with everything they have.

Because the alternative, accepting that their comfort is downstream of other people's dispossession, is not a policy position.

It is an identity catastrophe.

Imran Khan and his wife are being mentally tortured via prolonged solitary confinement for 22 to 23 hours a day in Pakistan Jail!

In addition, there is No television, No books and No meetings with family, personal doctors and political party members.

A lawyer sent to visit Imran Khan by Supreme Court for fact finding termed this torture on Imran Khan as "shocking and alarming" and attributed this to the Jail Superintendent SSP Sajid Baig who is widely known to take his orders from someone in Asim Munir’s team (army).

The Trump administration’s friendliness with Pakistan’s current regime isn’t accidental—it’s strategic. Keeping Imran Khan behind bars serves their interests, especially since “he’s seen as a bigger threat than Khamenei” as mentioned in the Epstien files.

At the same time, Asim Munir is using Khan as leverage to strengthen his own position.

Notice the timing: as Middle East tensions rise, Khan’s imprisonment tightens and his voice is further restricted. A free Imran Khan would be one of the strongest voices in the Muslim world—and that’s exactly what they don’t want.

You can take a principled position in support of the ceasefire, and you can also take an equally principled position against the actors involved, given their backgrounds, intentions, and history.

If this is an honest question, I’d say: Americans are rooting against America because we facilitated a genocide and followed it with a surprise attack on a girls elementary school followed by attacks on universities, medical centers, more schools, a world famous pharmaceutical research center, a volley ball team, an unfinished bridge we claimed was transporting weapons and then a nuclear power plant.
