#GI#GIbleed#urea
Elevated BUN: creatinine ratio favors UPPER over lower GI bleed.
Why?
1) Hb in stomach --> amino acids (aa).
2) aa absorbed by small intestine.
3) aa transported and taken up by liver.
4) In the liver, aa undergo deamination, leading to release of NH3.

1/4 - The Mystery
You are admitting a patient with confusion. As part of the initial work-up, an ECG is obtained.

DOACs in cancer-related DVT/PE
RCTs comparing apixaban (Caravaggio), rivaroxaban (selectD) & edoxaban (HokusaiVTE) with LMWH find comparable outcomes. But the 3 DOACs have important differences (this🧵). Apixaban is often the best choice in the broadest range of cancer situations

This is a question with a complex answer. This thread is only meant to scratch the surface and is a follow-up to a recent tweetorial on the use of epinephrine for cardiac arrest.
