For every PGY1 thats nervous about July 1, theres a PGY2 nervous about no longer being an intern, a PGY3 nervous about being in their last year, and a graduate nervous about being a new attending. And someone with years of experience out there who knows you all will be ok.

Calling a consult can be challenging, especially in the beginning of your intern year. You may get nervous. Follow these steps for a smoother experience.
1. Start with this sentence: โ ๐ฉ๐ฆ๐ญ๐ญ๐ฐ ๐ฎ๐บ ๐ฏ๐ข๐ฎ๐ฆ ๐ช๐ด (๐บ๐ฐ๐ถ๐ณ ๐ฏ๐ข๐ฎ๐ฆ), ๐ง๐ณ๐ฐ๐ฎ (๐บ๐ฐ๐ถ๐ณ ๐ด๐ฆ๐ณ๐ท๐ช๐ค๐ฆ). ๐ ๐ฉ๐ข๐ท๐ฆ ๐ข ๐ค๐ฐ๐ฏ๐ด๐ถ๐ญ๐ต ๐ง๐ฐ๐ณ ๐บ๐ฐ๐ถโ โฆTHATS IT. Nothing less nothing more. Usually they will respond by saying โok, please tell me last name, MRN, etcโ. The good thing about this approach is that you have informed them that you have a consult and then given them time to collect themselves. They may be sleeping, running a code, intubating someone, finishing a 24 hour shift. Anything is possible! Give them time to grab a pen and paper.
A lot of the times interns jump right into presenting HPI without giving the person in front any time. Please donโt do that. โ
2. Know exactly what you expect from the consulting service. This comes with time and experience so ask your senior residents about what do I expect. If you are calling cardiology about a concerning EKG findingโฆmaybe talk about that more rather than explaining their remote past surgical history. If youโre calling surgery for a laceration repairโฆmaybe donโt explain a lot about their irrelevant social history. Be succinct and to the point. Everyone is busy and everyone will appreciate you being succinct.
3. Know the urgency of the consult. If it is something that can wait till the morning then do it in the morning. If it is a chest pain with new ST elevations and troponinโฆthen it is probably worth calling cardiology at 3am. Ask your seniors the urgency of the consult.
4. Always ask the name of the resident/fellow you talked to for documentation purpose.
