Se acaban de publicar en JAMA los resultados del ensayo clínico FAST sobre bacteriemias por Gram negativos. 🧬 Evaluaron si obtener AST rápido mejora la evolución de los pacientes. 🧵👇

1️⃣ El Reto: Las infecciones en sangre por Gram negativos -incluidas cepas resistentes- son una carrera contra el reloj. El estudio analizó si disponer antes de los resultados de sensibilidad mejora el ranking de desenlaces clínicos (DOOR) a los 30 días.

2️⃣ El Hallazgo: La prueba rápida NO fue superior al método estándar en el desenlace clínico principal (48.8% de probabilidad de mejor resultado). 📉

3️⃣ La Buena Noticia: ¡La velocidad importa! ⚡ El grupo de AST rápido logró ajustar el tratamiento (escalar o desescalar) 14 horas antes. En infecciones con resistencia a carbapenémicos, el tiempo hasta una terapia eficaz bajó de 28 a 9.5 horas. ⏱️💊

4️⃣ Aunque no cambió la mortalidad general a 30 días, disponer antes de los resultados de AST es una herramienta clave para el uso responsable de antibióticos y la medicina de precisión. 🏥⏰🎯
