Interesting case
Middle aged man, HEMS primary for OOHCA. ROSC with anterolateral STEMI.
Thrombolysed (distance/time) and transported 2hrs to PCI centre and underwent rescue cath. 95% LAD.
Needed secondary transfer due to intercostal injury exacerbated by DAPT, heparin, lysis.
@MStott88 In an ideal world inpatient teams would only see people with a definitive work-up and some certainty about diagnosis/disposition if not time-critical. But with hospital overcrowding/access block the system must evolve to give better care "to the many". It's a hard balance

Patients want more tests, other clinicians are more comfortable with more tests, incident reviews almost never support less tests, and in Australia we don't have much litigation... yet
It's hard.
