Staff Sgt. David G. Bellavia earned the Medal of Honor for actions on Nov. 10, 2004, in Fallujah, Iraq, during Operation Phantom Fury.

His squad was trapped in a house by heavy enemy fire from under the stairs. He entered alone, laid down covering fire so they could escape, then went back in.

He assaulted an insurgent prepping an RPG (killed one, wounded another), cleared a dark room fighting two more at close range, pursued another up stairs, and shot a fifth leaping from the roof—single-handedly clearing the house, killing 4 and wounding 1. All with total disregard for his own life.
