Treatment of Hypothyroidism that Contains T3 is Associated with Reduced Risk of Dementia and Mortality.
Results from 1.26 million patients with hypothyroidism, 126 health organizations across 17 countries. https://t.co/7amwGgpTpN
Massive goiter (left) versus a control thyroid (right)!! The goiter is due to inactivation of the thyroglobulin gene. It compensates for inefficient T4 production in dead thyrocytes, thereby keeping T4 levels normal. Kudos to the Arvan group!! https://t.co/9v3Oig6CJU
In Vegas for AACE2026 (@TheAACE). I am impressed with how much this group has grown since Paul Jellinger, Jeff Garber and others started it. Congratulations!!
@LipidSniper@PeterAttiaMD The ratio should be customized for each patient; in general you decrease the dose of LT4 by 25 mcg and introduce LT3 5 mcg. Adjustments follow based on symptoms; TSH levels must remain within normal range.
In episode #373 of The Drive, I sit down with Antonio Bianco, M.D., Ph.D. (@bianco_lab), a world-renowned physician-scientist and expert in thyroid physiology and metabolism. Antonio explores the complex biology of thyroid hormone production, conversion, and regulation.

Thyroid hormones are important for neurodevelopment during the first trimester of pregnancy.
Here, @FedeSalasLucia@Bianco_Lab & team @UChicago model suboptimal thyroid hormone levels during the early stages of cerebral cortex development, revealing an unappreciated role for T4 in promoting early fetal cerebral cortex development: https://t.co/o4EG5GunNM
Preexisting hypothyroidism in 5,700 patients undergoing bariatric surgery increases the risks of short and long term adverse outcomes. Risks were higher in patients who underwent gastric bypass https://t.co/kJJBCb6uDj
@LipidSniper It is due to a disconnect between the D2 in the pituitary and in the other tissues. Exogenous T3 improves. We are submitting that paper soon.
We followed 105 patients before and after developing hypothyroidism (on LT4). Those patients with lower T3 were at risk for dyslipidemia (47.5% vs. 27.3%); even after adjusting for confounders (adjusted OR: 3.6). Monitoring T3 levels should be considered.

@vegananne@ITC2025 Yes, we have a study under review showing that LT4-treated patients have 30-40% higher risk of developing acute and chronic kidney disease; risks are higher with elevated TSH levels.

@EnajaratG The findings are valid only for patients with hypothyroidism. The association between treatment that contains T3 with reduced cases of dementia is very significant. There are between 15 and 20 million patients with hypothyroidism in the US.
