Since we launched #scaleupbp home monitoring in GP Practices in @NHSGrampian, we now have 55% of practices using home BP and in the last 12mths have clocked up >4,380 patient remote monitoring pathways and >5,380 avoided F2F apmts as a result. @MoragHearty @Brianmckinstry Result!
