Two biggest wins for me were not frameworks or skill bundles or software factory patterns (though surely there are good ones), but two simple concepts in my AGENTS.md (or CLAUDE.md). Roughly:

_for every feature request:_
- update the spec and ensure it remains consistent, eliminating inconsistencies and gaps, asking for more user input where necessary
- write a failing test before planning the implementation
- plan
- implement until tests pass
