It was a great pleasure to welcome GPs from across the UK to Imperial College’s Annual GP Teachers Conference today. General practice has a key role in the medical curriculum. It is where students see patients as individuals living with health conditions.

Primary care provides opportunities to learn about areas such as prevention and health promotion, the early diagnosis of illness, continuity of care, and the management of frailty and multimorbidity. These are increasingly important aspects of healthcare as populations globally age and the burden of long-term conditions increases.

Students also gain insight into the wider determinants of health and the importance of working with patients, families and multidisciplinary teams to deliver person-centred care. These experiences are essential in preparing future doctors for the realities of modern clinical practice, whatever specialty they ultimately choose.

This teaching takes place against the backdrop of a very challenging environment for primary care in the NHS. General practice is facing increasing demand, workforce pressures, rising complexity of care, and constrained resources. The fact that practices continue to support medical education despite these pressures is a testament to the professionalism and dedication of GP teachers and their teams. We are deeply grateful for your ongoing commitment.

The quality of medical education at Imperial depends on partnerships with practices and clinicians. By teaching our students, you are not only shaping the next generation of doctors but also helping to ensure that the values of holistic, patient-centred and community-based care remain at the heart of medicine.

@Azeem_Majeed Shifting healthcare out of hospital is just rhetoric without adequate funding, premises and staff. Right now, we are firefighting with fewer GPs per patient, growing list sizes and rising complexity. Prevention doesn’t happen in 10-minute slots when GPs are drowning in demand.

The key indicator of the government’s commitment to primary care is the proportion of the NHS budget spent on general practice. This remains well below the level it was 20 years ago. A shift towards prevention and community based care won’t happen without adequate funding.

These "interventions" by David Cameron and Rishi Sunak however well intentioned are now verging on irresponsible. There is good reason why activism even by ex PMs should not dictate national screening decisions. As a surgeon, researcher and recent health minister I'll explain why......

Screening for any disease is not a risk and harm free exercise both in terms of harms of the test itself and the complications of unnecessary treatment. That is why they are carefully balanced decisions based on whole populations not individual case studies as emotive as they might be.

Current studies and evidence from international trials in prostate cancer are not representative of the UK population which has higher numbers of black men who are indeed at higher risk of prostate cancer but need evidence and data tailored to their needs.

What we need is better modern screening tools rather than a blanket application of suboptimal tests which may miss cancers and lead to unnecessary treatment. And there is a clinical trial ready to answer exactly these questions set up by some of the brightest minds in the UK and backed by the @NIHRresearch and @ProstateUKVoice https://t.co/ExdKV1MOMQ

So please do use your activism to push politicians and researchers to answer these questions faster, to break the mould, to push the boundaries to get the science done. But do not inadvertently become complicit in an uncontrolled screening experiment of men and black men because of your desire to do “something”.

@Azeem_Majeed In primary care, every day we see patients with multiple long-term conditions, social complexities, mental health issues or vague, evolving symptoms that don’t fit neatly into a single pathway. We need systems that support and protect that judgment, not replace it.

Standardisation of care pathways works well for patients with straightforward problems. But medical practice is often complex and non-linear. Good clinicians use guidelines as a foundation for clinical practice and then add clinical judgment, experience and adaptability.
