@ShoulderGeek1
Hi Jo,
Iโm hoping I can pick your brain.
In my last clinic before Christmas (always the way) and Iโve had an unusual shoulder case that I have some concerns about:
๐ธ35 year male, woke up with R shoulder pain 6 weeks ago with no apparent cause (1 of 4)
@ShoulderGeek1 Ah of course, thank you!
That fits pretty much spot on.
Iโm in an area with spiralling COVID rates. Given that he is immune suppressed is there any value to further investigation at this time?
My inclination is to remotely manage for the moment.
๐ธHe is immuno-compromised due to having had his spleen removed following a motorcycle accident 2017, he take antibiotics daily.
๐ธ(Grandfather has rheumatoid arthritis)
Thinking supra-scapula nerve, but it is the WHY that concerns me.
Any thoughts? (4 of 4)
externally rotate (in different positions)
๐ธHeโs well within himself, no night pain, no recent illnesses
๐ธClear medical history except....

@TaylorAlanJ@consultantPT Sometimes I think the concept of high value care gets skewed.
If youโd been an NHS patient and a simple bit of tape helped you sleep you would have been delighted with the service.
If youโd paid to see a practitioner privately you would have thought it well worth the money.
@HannahMorleynhs@AdamMeakins Iโve lost count of the number of times I have requested an x-ray for suspected frozen shoulder.
I do however know exactly the number of times I have had a request accepted......0!
@Brad_Neal_07 @TaylorAlanJ We are always quite tough on clinicians when we see headlines such as this, but do we really know that these beliefs were caused by clinicians?
@tomgoom Wait....what? How am I only just finding out about this now?
Weโve only been on lockdown a week and Iโve already lost the fundamentals of seasonal time keeping. ๐คฆโโ๏ธ
@GregLehman And thatโs why no matter what we say or do, how much we publicise, how much we educate, passively modalities/treatments will always be the thing that that the majority of people seek.
@AdamMeakins Iโve been through a range of emotions and concerns of late. Ultimately, it was a relief to close the clinic doors.
Virus or no virus, I genuinely think online consultations are the future.
Iโm actually excited about reinventing my practice.
I am a doctor.
My wife is a doctor.
We have two children.
When calling on authorities to #CloseTheSchools, please consider that, if all schools and childcare shut due to #COVID19, one of us will not be able to work.
@function2fitnes Benoy it's like we we talking about on your Venn course in January, there are so many issues with injections being performed without a clinical reasoning process.
Iโm always concerned about consent here, is the patient making an informed choice? (Often I think not)
@adamdobson123@FlippinPain@RogerHilfiker Yes bruising, but is it surprising?
We know from our clinics that if you directly challenge someoneโs beliefs often the barriers will come up.
To be successful we have to get the public to buy in to our way of thinking, same as we do in clinic.
@PeteOSullivanPT LBP pt recently, multiple sessions, multiple therapists, told SIJ was out.
Anxiety, depression, too scared to ๐๐ปโโ๏ธ
Convinced her to do a 10 minute ๐๐ปโโ๏ธ, she felt good.
Your messages get through to some of us and we pass it on to our patients. Keep up the good work!
