No, it’s not a Critical Factors meeting. Some of our team in our main roles fetching another huge load of PPE to support healthcare professionals keep safe.
Other industries recognise the value in training their teams to #communicate effectively, recognising the importance of getting the message through and checking for an appropriate response.

Effective communication can be challenging, as it requires conscious effort to ensure the message is clear for others, has been received and acted upon.
#humanfactors#Communication is so easy in theory but a real skill in practice.

Poor communication is a significant cause of medical error
In 2022-3 it was commonest cause of never events such as wrong site surgery, implants or protheses, and retained instruments/swabs
Our latest case of the month highlights poor communication

So important to include all the team in a culture of #patientsafety . Everyone has a role to play and a contribution to make. So important we value the contributions & heed the concerns of all the team.
🆕We've published the first in a series of reports that look at #NHSWorkforce & #PatientSafety. Our safety recommendations to @NHSEngland aim to increase the engagement of temporary staff in patient safety investigations and provide them with more support. https://t.co/419V2Yajsa

In aviation we take it for granted that events can be unraveled to understand what happened and why, with comprehensive investigation and learning outcomes.
Behavioural #humanfactors typically feature heavily, both as contributors & safety barriers.

Certainly not the 'planes we fly'.
Looking fwd to observing more robotic #surgery tmrw (not @ hospital cited) for us to share ideas & collaborate on common #humanfactors challenges of #teamwork, maintaining #situationalawareness & #communication.
Last week saw us deliver on an innovative educational solution to embed #humanfactors in foundation training for #dentistry. Huge thanks to all collaborators working towards #patientsafety and #teamwork in dentistry and dental education.
Amused to be asked if we were no longer in practice as we hadn't posted for a while. In fact we've been too busy with #humanfactors projects in #dentistry#maternitysafety and #meded to get round to it.
Huge thanks to those involved.
Tragic case.
Often there's no single root cause with holes in many layers that should provide safety barriers.
How many of us resort to improvisation/workarounds to get the job done?
Whenever something is unusual, it's worth pausing to check with fresh eyes..

Inadvertent drug error causing death.
Levomepromazine instead of Levetiracetam.
Error occurred despite two person check:

Thank you for the invitation to speak @NottmMaternity …an honour to be invited. An amazing event, so well organised !
It’s a huge privilege to play our part in your #culturechange journey showcasing how behavioural #humanfactors is such a key component.
Last night I attend @NottmMaternity’s first cultural event evening! Huge thanks to the very wonderful Dr Lucy Blanks for organising and all those who attended! Great to see you all and here’s to being part of the change…we’ve got this ❤️💪🏻🤩 @CriticalFactors@PMAs_NUH @TeamNUH
This. Don’t come to me with an ‘improvement’ for safety or otherwise if it doesn’t also REDUCE workload and/or task complexity for those providing patient care.
How we breathe can have a huge impact on our human performance. Many of us don’t think about it or find it hard to believe.
But it’s easy to adopt without others even noticing.
Some interesting examples and techniques here from a surgeon.

Great to hear delegates share how learning from the Core has opened their eyes to what they see in their working environments.
Thank you to all involved, it’s a privilege to be involved.
Another multidisciplinary group of #healthcareprofessionals completed our Advanced #HumanFactors course this week, completing 4 days in total of intensive, immersive training. Well done to all involved 👍
UK space launch: Historic Cornwall rocket launch ends in failure https://t.co/nNm6C9k8VB
#failure is an emotive word.
Useful for attention grabbing headlines, but undermines all the successful collaborative effort and useful lessons that can be learned. #humanfactors#teamwork
I’ve never observed anyone, regardless of field, achieve lasting prominence while voicing rancor or focusing much on the failings of others. Create and share, support others and enjoy. Givers and creators always prevail.
We’re looking for Maternity Investigators to join our team. There are two vacancies live on #NHSJobs, for those from clinical and non-clinical backgrounds. Our Maternity Investigators are home based. Apply now! https://t.co/XnufaciFHE
