They are using it as a shorthand to ridicule someone's non-existent sex life, so they presume that the person is an incel in a literal sense. You don't call a "chad" an incel. Unless you use it wrong I guess.

It literally only means "involuntary celibacy", so a person who has no sex life despite their efforts. And the main reason for that varies per person, but usually it's either because the person is anti-social, or herbivorous.
