I recall in TN, #6 went down on day 1 of practice. It ultimately was ruled pilot fatigue. Mistakes happen, but they ARE rare with BA. Safety is taken into each maneuver as outlined here.

I was at my local @AceHardware where they were flying a huge Old Glory hanging from the building - except they had it backwards. I spoke with the clerk and she was horrified and embarrassed and said they'd correct it right away.
I drove by 30 min later and it was fixed. This couldn't have been a easy task with it being 30' in the air. The fact that they jumped on it so fast was impressive.
