On July 6, CMS quietly did something no Medicare Advantage plan has ever done… it started handing out exemptions from “prior authorization” (WISeR isn’t really prior authorization) to the physicians who earn them.
The WISeR Model, the Wasteful and Inappropriate Service Reduction demonstration CMS launched in Original Medicare on January 1, 2026, has become one of the most misunderstood programs in Medicare policy. Democrats in Congress have tried twice to stop it, most recently with a Congressional Review Act resolution that came to a vote and failed in the Senate. Critics call it AI-driven care denial, which makes sense. Any time CMS puts a for-profit contractor and artificial intelligence in the same sentence as a Medicare coverage decision, critics are right to be skeptical.
WISeR is not prior authorization the way independent physicians have experienced it under Medicare Advantage for the last decade. It does not let contractors invent their own coverage criteria. It does not look at every claim, only those with high levels of prior fraud/waste/abuse. In addition, as of July 6, it now formally exempts physicians who consistently get it right (gold carding).
This week I will walk through what WISeR actually does, mechanically, and make the case that independent physicians should be pushing Congress to build on it rather than repeal it. The real target for reform is not this model. It is every private Medicare Advantage plan that has never offered physicians anything close to what WISeR just built.
Every tumor board runs on the same hidden tax: someone spends hours pulling the case together before anyone in the room gets to actually think.

These tools can be used by anyone anywhere. Not only for physicians in the US and Europe. LMICs need clinical decision support tools as well.
The point is not to replace clinical judgment. It is to move the human effort from data collection to decision making, and to make sure trial options show up at the start of the conversation instead of never.

Knicks fans screaming about the league not retroactively upgrading Victor Wembanyama’s foul should actually be counting their blessings tonight.
Why? Because while they were focused on Wemby, I'm told the NBA league office was reviewing this entire bench clearing sequence and specifically the actions of Jose Alvarado who previously served a major three-game suspension for having been involved in a nearly identical bench-clearing incident in 2024 against the Miami Heat.
When the scuffle kicks off under the basket, several Knicks players leave the bench area in violation of the rules and Alvarado gets right in the middle of the scrum.
He is seen pushing, heavily engaged with both Luke Kornet and De'Aaron Fox, waving his finger and escalating the tension getting face to face with Fox.
Under the NBA’s strict bench-clearing rules, he is undeniably not acting as a "peacekeeper."
The Knicks players left the bench before San Antonio called timeout.
If the league had decided to apply the letter of the black and white law regarding bench conduct and physical escalation, as they have multiple times this season, New York could easily be down a crucial backcourt piece or 4 for Game 4.
Take the Wembanyama non-upgrade as a wash and be thankful, because the Knicks dodged a massive suspension bullet here. 🧵
