It is not a one sided issue. 50% or more responsibility is for the HOD and teaching staff of the institution . They have to make sure that the academic schedule goes on uninterruptedly on all days except Sunday. SRs must be given excellent training in clinical cardiology, echo and intervention. Equal importance to all aspects of cardiology. While in OPD give them cases and ask specific things like JVP findings, Palpatory findings, auscultation findings etc etc. Make them fit for facing OSCE and viva. Show them every interesting ECG, X-ray and ask them. If a cath for Constriction is posted, pick up one SR and ask them to do the cath and you scrub in with them and go on asking questions while he is doing, correct him and make them come prepared. Make sure that they are capable of doing primary angioplasty independently by final year. Make sure that they have minimum 3 publications before finishing the course. Before exam they must write their thesis as paper and must be submitted to a journal for me to sign the application form. Give them Sunday off. Give them the leaves they are permitted by NMC promptly. Consider them as your kids, love them and be strict to them. Even some have approached me for personal matters for advise.
During seminar the first slide must be references. No cut copy paste. I have told them that at least 15% of things they are presenting must be new information for me to be rated as adequate. If substandard presentation, I simply walk out of seminar room without shouting. Any lapse in patient management will be serious matter here, no compromise.
Take them for coffee whenever free and I never permit them to pay any bills. No costly treats by them even after passing the DM exam.
No entry to class after 8.15 Am, the seminar room will be locked by then. Communication to bystanders must be meticulous, if anyone’s bystander approach me for knowing patient’s condition I will ask the SR why that happened. Evening rounds must be taken daily in ICUs and wards and must be communicated to me.
Make sure they pass in practicals.
It runs wonderfully well here sir. To my knowledge no one hates me.
The only thing more difficult than doing a second residency is returning and exchanging clothes with @knyamed@abhijeetkaji .
The app is slow and I have to sit and talk to a AI bot in all my free time
Living in a nursing home shouldn’t mean giving up everyday freedoms.
I just signed a bill allowing seniors living in nursing homes to consume alcohol - so that everyone can enjoy happy hour!
If your rank is good enough to do endo at aiims do medicine first
Medicine is too vast to decide at a post mbbs level
I changed my mind 5 times during residency
They should not get paid less just because we are getting paid less.
We should learn to ask for more.
To the large corporate hospitals there is no difference between a fellow and a daily wage labourer.
They are trying to extract as much work in as little money as possible.
I agree completely
MD physiology guys should be allowed to DM cardiac and neuro anaesthesia and critical care.
And MD anatomy should be allowed to eligible for all surgical super specialities.
Har ghar super specialist yojna💪
MD microbiology and Biochemistry should be eligible for DM infectious diseases and Endocrinology respectively.
Whatever reasons now quoted against this is just an ego trip of a particular speciality.
Another common debate in Indian #MedTwitter
You can find diamonds in a pile a of shit and you can find specks of carbon in the finest diamonds
It all depends on the student
If he or she is average with no curiosity and no desire to improve
I doubt more lectures or better hands on training will do anything
Most good academic programmes - MD or DNB - tend to attract the 'best and the brightest'
These people will learn well in a cowshed under candlelight and will perform well in exams with minimum preparation
They are just that smart, intelligent and resourceful
So its heavily skewed towards producing well trained doctors
If you really want to see how good a programme is - MD or DNB
Take a group of really average and unmotivated students
We will see how well the programme works
Off topic
My opinion is that
MD or DNB Chest Medicine degrees should be phased out like several diploma programmes since pulmonology is a subspecialist subject now
Narrow broad specialties restrict choices in SS for people with these postgraduate degrees

No one on the internet is as insecure as anaesthesia folks unfortunately.
A beautiful branch but I don’t know whether it’s the lack of front facing work or because half the Fresh anaesthesiology residents would give up their left kidney for a medicine seat.
Anesthesiologists, Pulmonologists and Physicians make the best intensivists in that particular order.
Critical care skills doesn't come free by virtue of a degree in any of these three, it's a separate entity and a bigger game.
Tell me your doctor has an EMI to pay without telling me.
At this rate, we’ll be treating common sneezes with Meropenem by 2027.
All she needed was steam inhalation and some rest!
they don’t tell you this at the beginning of residency but this is exactly what happens in residency as you transition from a first year resident to a registrar :(
Anybody here who’s fuckin around with GLP1’s experienced zero appetite suppression? I have experienced zero appetite suppression - upon further investigation, my gene report suggested a BDNF gene variant that is prone to weakened satiety signalling. Anybody here an expert on this matter?
@MGMotorIn 30 visits, yet;
• No detail diagnostic report
• No warranty clause justifying denial
• No written repair estimate or consent
• No explanation of “water ingress” in an IP-rated HV battery
This raises serious concerns on transparency & safety.
