Selective biliary cannulation requires aligning the sphincterotome wire with the natural axis of the bile duct.

1/ Understand septum anatomy: the septum separates biliary and pancreatic lumens, dictating the angle of wire entry.

2/ Key cannulation principles: maintain en-face papillary position, gentle wire probing, and minimal thermal contact.

3/ The compact disc method uses small rotational adjustments of the duodenoscope wheel to match the 11 o'clock duct axis.

4/ When cannulation stalls, shift early to advanced tips: wire-guided access, double wire technique, or pre-cut papillotomy.
