@MichaelMwachu@JACCJournals That’s always a viable option. Pt Was deemed inoperable during heart team discussion that lead to TAVR. We are convinced that this was the best option vs. emergency sternotomy during cardiogenic shock.

- big annulus requiring an Evolut 34
- tight sinotubular junction (where the “seal” happens)
- mitral prosthesis super close to the annulus
- interaction TAVR-mitral prosthesis during deployment leading to embolisation
@PedroMDMSc@JACCJournals The embolized valve is stuck and interacts with the lower one (which is implanted correctly), tying to force it downwards could compromise the correctly implanted valve. The options were covered stent or surgery.
