Here's another real-life example of how PBMs are corrupt and harming healthcare in America.
Consider this situation: The same patient, same prescription, just one month apart. In April, the patient's copay was $35 for a generic Symbicort inhaler. In May, the patient was automatically enrolled in Caremark's mail-order program without his knowledge. Now, if he uses a pharmacy outside of their mail-order system, his copay will be $230.19.
This patient, who is 62 years old, has been a customer of my pharmacy his entire life and has severe COPD. This inhaler is a life-saving medication for him. When he came to pick up his inhaler, he was shocked by the copay and stated that he couldn't afford it and that he couldn't wait for mail-order. I explained to him that he can opt out of this program.
Finally, after an hour on the phone with his PBM, he was able to opt out of the mail-order program he had been automatically enrolled in and can continue using our pharmacy for his usual copay.
I can't help but think of all the patients across our country like this one, that are in need of their medications and get blind-sided by their PBM. The majority of these patient's do not have a personal pharmacist to explain their options. They just go without medicine.
This must stop now.

Patient that had triple bypass surgery comes into my pharmacy. He's confused about his new medications and is wondering what he is supposed to start and which ones to stop. I spent 20 minutes consulting the patient and coming up with a detailed plan for his medications moving forward. He was very thankful for my services, and I was glad I could help him.
Here is my reward for taking care of this patient --> -$33.05 loss on a brand name medication.
This is a Medicare plan, and it is flat out garbage. There is zero compensation for improving healthcare of patients.

The United States of America spends by far more money on healthcare than any country in the world, yet we are middle of the pack in regard to actual healthcare provided.
Why is that?
Four companies in the top 10 of the fortune 500 are healthcare companies. 🧐
CVS Pharmacy merged with Caremark (PBM - middleman) then acquired Aetna an insurance company to form the golden trifecta of vertical integration. Caremark their PBM facilitates over 20% of all the prescriptions filled in America.
By controlling reimbursements in over 1 out of every 5 prescriptions in America they've been able to underpay competitors (independents, Walgreens, etc.) and overpay themselves to grow exponentially. The persistent underwater reimbursements have led to the closure of thousands of independent pharmacies and as we've seen lately Walgreens as well.
It's not a coincidence that right after Walgreens was acquired by a private equity firm and closed hundreds of stores, that CVS unveils the final chapter of their plan. Now with so many pharmacies having closed, they will move into those areas and put in their own mini pop-up pharmacies.
CVS is an oligopoly that controls the majority of the market. Once they're in control of these rural areas in our country where they assisted in forcing independents to close, who will stop them from doing whatever they want? Limiting access to healthcare, overcharging prices, poor customer service, etc. There will be zero accountability and no one to compete against them.
Healthcare overall has suffered tremendously due to the corrupt practices of PBMs and will continue to do more damage with this move. When will congress step up do something?

Hey @AndyBeshearKY, why is your DOI Commissioner supporting companies listed on S&P 500 over citizens in our own state?
Below is an example of a pharmacy claim that I filed a complaint on. This patient lives in KY, works in KY, uses pharmacy services in KY. Yet, he's being forced into mail order, because the KY DOI deems this ERISA and that there's nothing they can do.
There are citizens that are losing access to rural healthcare because of this. There are independent pharmacies closing all over the state because of this.
Our legislators passed a very important law SB188 last year and your appointed DOI Commissioner is not enforcing it!
Do something about this!

As a Co-op you should be benefiting rural communities. By selecting Caremark as a PBM that prefers big chains (highlighted below), you are hurting rural communities that you claim you’re helping. Here’s an example of a prescription drug claim under your plan that was paid $80.62 under cost to a local independent pharmacy that supports that rural community. Change your PBM or change your mission statement. #pbm #pbmcorruption #coop
