A) ✅Pneumothorax is the most important complication of a thoracocentesis procedure.
📍Cause- It occurs when the lung is accidentally punctured by the needle, allowing air to enter the pleural space. This can➡️collapsed lung.
🩺C/F- shortness of breath, dry cough, chest pain, fatigue, ⬆️HR, Cyanosis (severe cases).
🖥️Dx- CXR.
⛑️Can be avoided by using USG & proper procedural technique.

Minocycline-induced pigmentation

Pathogenesis:

- Long-term minocycline use → deposition of pigmented metabolites (iron–minocycline complexes, melanin-like products) in skin, mucosa, teeth, nails.

- Accumulates especially in sun-exposed areas.

Clinical Features:

- Blue–gray or black discoloration of face, shins, gums, sclerae.

- May involve scars, acne lesions, and oral mucosa.

- Often after prolonged therapy (> months to years).

- Cosmetic concern but usually benign.

Treatment:

- Discontinue minocycline → gradual fading over months–years (sometimes permanent).

- Alternative antibiotic (e.g., doxycycline) if needed.

- Laser therapy in refractory cases.

FAQs:

- Is this dangerous? → No, mainly cosmetic.

- Reversible? → May persist even after stopping drug.

- Other tetracyclines? → Doxycycline rarely causes pigmentation.

- Prevention? → Avoid long-term unnecessary minocycline use; monitor skin changes.

Psoriasiform nemolizumab-associated dermatitis

Suspected mechanism: IL-31 inhibition suppresses Th2 but triggers compensatory Th17 response, causing psoriasiform lesions.

Patient was treated with IM methylprednisolone & nemolizumab discontinued, leading to resolution in 1 wk. Therapy was switched to RINVOQ®️ (upadacitinib), improving AD but causing JAK-inhibitor–associated acne.
