continuing empiric vancomycin for >48 hours without *any* evidence of MRSA (no PCR, culture, or gram stain data to support the presence of MRSA) should be a federal crime.
Okay you Med Twitter cefadroxil-ites! The long awaited cohort study of cefadroxil for bacteremia (Including staph aureus!) with or without osteo is here! Have at it!

@DiorIzzy@ABsteward@IDSAInfo There's actually a physics equation that describes this paradigm in the universe. It's known as the Schrodinger Guideline Formula:
shmidelines = arrogance * inertia (squared)
@MCPhillipsMD@DrToddLee@ABsteward So that's a really important point. The key metric isn't bone:serum. The key metric is, what concentration of drug can you get into bone (mcg drug/g bone) vs. the MIC. Check out Tables 1 and 2 in https://t.co/BjdVdH5hPP.
Also tables in WikiGuidelines on pyogenic osteo.
Today in @NEJM we show that cefazolin should be the preferred agent for MSSA bacteremia
https://t.co/BCVDzDfUqL
We also show in @TheLancet that penicillin should be preferred to anti-staphyloccal penicilins for PSSA

@BradSpellberg Would love for you to do a grand rounds on shorter is better or the humility of uncertainty! I'm a pharmacist trying to be more involved in the ID space at my hospital.
@ABsteward I’m working on a retrospective chart review study to try and validate local risk factors for MRSA/PSA in CAP at my hospital. Are there any risk factors I should include that I might be missing? Appreciate the input!
@DiorIzzy What are your thoughts on anaerobic coverage for possible aspiration pneumonia? Several providers are insistent on zosynq but I thought anaerobes were rarely isolated and ceftriaxone has coverage of normal mouth anaerobes and prevents unnecessary use of an anti-PSA abx?
@DrToddLee What are your thoughts on anaerobic coverage for aspiration pneumonia? Several providers are insistent on needing pip/tazo. Anaerobes are rarely isolated and ceftriaxone has decent coverage of normal mouth anaerobes preventing unnecessary use of an anti-PSA abx.
