Convening multiple stakeholders to deliberate on an integrated response to heat and Chronic Kidney and Metabolic Diseases (CKMDs) in India
The Heat and Health Initiative, with a focus on CKMDs is a consortium-led initiative ledby HRIDAY, the Healthy India Alliance/India NCD Alliance (HIA), in collaboration with the Public Health Foundation of India (PHFI), with national and state partners.
The Gujarat Multi-Stakeholder Consultation, held on 4 June 2026 in Gandhinagar, Gujarat, in partnership with the Indian Institute of Public Health Gandhinagar (IIPHG), marked the first in a series of consultations being convened under this initiative.
As we mark World Environment Day 2026, the consultation served as a timely reminder that climate change is increasingly a public health challenge, and that building resilience to extreme heat must become an integral part of protecting population health and advancing sustainable development.
The consultation brought together cross-sectoral stakeholders and experts from State Health Department, Government of Gujarat, Disaster Management Authorities, Heat Action Plan teams, Nodal Officers from the National Programme for Climate Change and Human Health (NPCCHH) and the National Programme for the Prevention and Control of NCDs (NP-NCD), medical colleges and research institutes, professional associations, civil society organizations/NGOs, State UN agencies, implementation partners, frontline health workers, and lived-experience community representatives.
Some key insights generated from the consultation:
1️⃣ Heat and CKMDs require stronger convergence across Heat Action Plans, NPCCHH, NP-NCD & related programmes
2️⃣ Vulnerable populations require targeted support, particularly outdoor workers, low-income communities, older adults & people with chronic conditions
3️⃣ Capacity building is needed for health professionals, frontline workers, programme managers & communities
4️⃣ Existing heat alert systems offer opportunities for targeted CKMD-focused advisories
5️⃣ Stronger surveillance is needed for heat-related cardiovascular, kidney & metabolic health outcomes
6️⃣ People-centred care pathways can support preparedness & continuity of care during extreme heat
7️⃣ Technology offers opportunities to strengthen heat-health preparedness, response & monitoring
8️⃣ Early detection & preventive action can reduce morbidity and improve resilience
9️⃣ Lived experiences provide valuable insights for designing responsive & equitable interventions
Our heartfelt thanks to all distinguished speakers, panellists, moderators, and thematic leads who enriched the consultation through their expertise, insights, and commitment to advancing heat-resilient health systems.
We look forward to continuing this collective journey and translating these discussions into stronger partnerships, informed action, and resilient health systems for the future.

"Insulin is not a luxury, it is the difference between life and death."
Meet Amrita Rupani, representing the Yog Dhyan Foundation in Delhi and serving as a HRIDAY Type 1 Diabetes Ambassador.
Through her years of dedicated work supporting children and families from low-to-medium-resource communities, Amrita has seen firsthand how Type 1 Diabetes impacts the entire family unit. She reminds us that managing a lifelong health condition should never feel like an impossible financial burden.
That is why we proudly stand with Amrita in saluting the Government of India and the Ministry of Health and Family Welfare (MoHFW) for the historic release of the Guidance Document on Diabetes Mellitus in Children!
This newly integrated framework guarantees a comprehensive, free-of-cost care package at public health facilities, including essential lifelong insulin, glucometers, testing strips, and seamless follow-up care.
As Amrita beautifully emphasizes, this breakthrough ensures that no family will ever have to make the heartbreaking choice between a child's life-saving medicine and a basic meal.
When we provide our children with the right care and equity, they don’t just survive, they run, they serve, and they lead.
Thank you, MoHFW, for this powerful step toward healthcare for all. To every family navigating T1D across India, dignity and support are finally standard care.

Strengthening community engagement and person-centered care for people living with Multiple Long-Term Conditions (MLTCs) requires listening to communities, building healthcare provider capacity, and adapting interventions to local contexts.
During a 5-day field visit to Kathmandu, Nepal, as part of the NIHR Global Health Research Centre for Multiple Long-Term Conditions, teams from Kathmandu Medical College and HRIDAY collaborated with community members and healthcare providers to strengthen Community Engagement and Involvement (CEI) activities in Dadhikot and the Urban Health Promotion Centre (UHPC), Ward 27.
The visit focused on three key areas:
➡️ Community engagement through street play and microdocumentary screenings, creating space for discussions on MLTCs, nutrition, alcohol use, healthy lifestyles, and the role of Community Champions.
➡️ Capacity building of healthcare professionals through training on self-care, empathy, cultural competency, CEI, and effective health communication.
➡️ Strengthening implementation readiness through CEI planning, storytelling discussions, and CRT Delphi activities to support context-specific research and community-centered interventions.
A key takeaway from the visit was the importance of culturally relevant communication, meaningful community participation, and empathetic healthcare delivery in improving care for people living with long-term conditions.
The visit also highlighted valuable community insights, including the need for locally adapted resources, greater awareness of referral pathways, and opportunities to promote healthy behaviours through activities such as yoga and physical exercise.
Together, these insights are strengthening CEI implementation and fostering more responsive, community-informed care in Nepal.

"Insulin alone is not enough... We need to equip families with the right education, support, and dignity."
We are incredibly proud to share this message from Riddhi Modi, design entrepreneur and Type 1 Diabetes Lived Experience Ambassador, reflecting on a landmark achievement for healthcare in India.
The Ministry of Health and Family Welfare (MOHFW) has officially released the Guidance Document on Diabetes Mellitus in Children! This historic initiative integrates childhood diabetes care directly into India's public health system, ensuring:
Free-of-Cost Care Packages: Lifelong insulin, glucometers, strips, and diagnostics at public health facilities to ease the financial burden on families.
An Integrated Continuum of Care: Seamlessly connecting community-level screening with district hospitals and advanced medical care.
Early Detection via the "4Ts": Spreading awareness about the warning signs - Toilet, Thirsty, Tired, and Thinner.
Family Empowerment: Moving beyond clinical treatment to provide structured training, emergency response tools, and emotional confidence to the entire family.
As Riddhi beautifully puts it, a T1D diagnosis impacts the whole family, mothers, fathers, and siblings alike. This guidance document is a massive step toward ensuring that every child and family navigating this journey feels seen, supported, and equipped.
A huge thank you to the Ministry of Health and Family Welfare and everyone involved in making this a reality. Together, we are sending a powerful message to every T1D family in India: You are not alone.

Voices of youth echoed through Kartavya Bhawan-1, New Delhi, during the Ministry of Health & Family Welfare's observance of World No Tobacco Day 2026.
As part of the event, HRIDAY Youth Ambassadors from Tagore International School, New Delhi, performed the powerful nukkad natak "नया रूप, पुराना ज़हर" (New Form, Old Poison), raising awareness against vapes, e-cigarettes, hookah, and nicotine pouches, and exposing how addiction through new products and marketing tactics are being reinvented.
Aligned with the WHO World No Tobacco Day 2026 theme, "Unmasking the Appeal: Countering Nicotine and Tobacco Addiction," the performance highlighted how products marketed as modern, fashionable, or "safer" alternatives often conceal the same addictive substance: Nicotine. Through compelling storytelling, the youth performers challenged myths surrounding tobacco use (all forms), vaping and other emerging nicotine products while encouraging informed choices and healthier futures.
The performance was witnessed by Health Secretary Ms. Punya Salila Srivastava, Dr. L. Swasticharan, Deputy Director General, Directorate General of Health Services, Shri Prabhakar, Joint Secretary (Tobacco Control), along with senior officials from MoHFW, WHO representatives, civil society organizations, and other key stakeholders.
HRIDAY is proud to support youth-led advocacy and empower young people to become champions for a tobacco- and nicotine-free future. Because when young voices speak up, they help unmask the truth behind addiction and inspire lasting chang

Behind every flavoured vape, candy-coloured pouch, and "lite" cigarette lies one truth the tobacco and nicotine industry is selling addiction to our children.
The reality we cannot ignore:
➡️ Tobacco kills nearly 8 million people worldwide every year including over 1.35 million Indians, that's more than 3,700 deaths every single day. (WHO; ICMR 2020)
➡️ 28.6% of Indian adults (over 267 million people) use tobacco in some form. (GATS-2, MoHFW 2016-17)
➡️ Globally, 40 million adolescents aged 13–15 already use tobacco, and 15 million use e-cigarettes. (WHO, 2026)
➡️ Adolescents are, on average, 9 times more likely to vape than adults, a generation being deliberately hooked. (WHO 2020)
For over two decades, HRIDAY (Health Related Information Dissemination Amongst Youth) has stood at the forefront of India's tobacco control movement.
This World No Tobacco Day, let's:
✅ Expose the occupation tactics - flavours, social media marketing, "harm-reduction" myths
✅ Protect children and adolescents from tobacco and nicotine addiction
✅ Empower youth as agents of change
✅ Enforce COTPA, FCTC and the ENDS ban
Support every person trying to quit

The Butterfly Effect - On Your Health.
It is butterfly-shaped, sits at the base of your neck, and weighs barely 25 grams yet your thyroid silently regulates your metabolism, heart rate, mood, energy, fertility and more. When it goes off-balance, the whole body listens.
Why this matters for India:
➡️ An estimated 42 million Indians live with thyroid disorders. (NFHS-V; 2019–2021)
➡️ 1 in 10 Indian adults is affected by hypothyroidism compared with around 2% globally. (The Lancet Diabetes & Endocrinology 2014)
➡️ Thyroid disorders are the second most common endocrine disorder in India, after diabetes. Women, especially over 40, are disproportionately affected yet many remain undiagnosed for years (NFHS-V, 2021; Thyroid Research and Practice, 2022).
➡️ Congenital hypothyroidism affects roughly 1 in 1,000 newborns in India, three to four times higher than in Western countries making newborn screening critical. (Indian Journal of Endocrinology & Metabolism, 2024)
The good news?
Most thyroid disorders are easily detected with a simple blood tests and effectively managed with timely treatment.
Adequate iodine through iodized salt used in moderation (≤5g/day, as per WHO) - remains one of India's most successful public health interventions, while early screening, especially during pregnancy and across midlife, can transform outcomes for millions of women and children.
This World Thyroid Day, take 5 simple steps:
➡️ Get tested - a TSH blood test is quick, affordable, and conclusive
➡️ Use iodized salt but keep total salt intake under 5g/day
➡️ Screen during pregnancy and for every newborn
➡️ Know the symptoms - fatigue, weight changes, hair loss, mood shifts, cold intolerance, irregular periods
➡️ Talk about it - break the silence, especially in families with a history of thyroid disease
Thyroid health is a window into how India is living, eating and ageing. Let's listen to that window.

This World No Tobacco Day, HRIDAY is putting the mic in your hands.

High blood pressure is the world's leading silent killer and it's quietly claiming lives across India every single day.
The numbers we cannot ignore:
➡️ Over 294 million people in the WHO South-East Asia Region live with hypertension. (WHO SEARO, 2025)
➡️ In India, roughly 1 in 4 adults has hypertension yet only about 12% have it under control. (India Hypertension Control Initiative / WHO India 2022)
Hypertension drives 54% of stroke and 47% of ischaemic heart disease globally. (Indian Journal of Public Health 2023)
At HRIDAY, our work on Non-Communicable Disease (NCD) prevention and control, through research, meaningful youth and community engagement, and as the Secretariat of the Healthy India Alliance (India NCD Alliance) has long recognised hypertension as a cornerstone of India's NCD burden.
From promoting for salt reduction and healthier food environments to mobilising youth and people living with NCDs as agents of change, we believe prevention begins with awareness and ends with action.
This World Hypertension Day, take 5 simple steps:
✅ CHECK your blood pressure regular
✅ EAT a balanced diet, low in salt
✅ MOVE - be active every day
✅ MAINTAIN a healthy weight
✅ MANAGE stress and sleep well
Know your numbers. Protect your heart. Because a healthier India begins with each one of us.

Sunita, a security guard from a small village in Jodhpur, was managing more than just her job and household. Over time, she was diagnosed with diabetes, hypertension, and COPD.
But her real struggle wasn’t just medical. It was losing her husband, withdrawing from life, and trying to make sense of multiple illnesses while navigating a system that treated each condition but not her as a whole person.
She was prescribed medicines, advised follow-ups, and given instructions. Yet, like many patients, she struggled with adherence not because she didn’t care, but because no one had fully understood her reality.
Everything changed during a community engagement session. For the first time, she was not just told what to do she was heard. She asked questions, reflected on her journey, and slowly began to regain control over her health.
This is the gap we often miss in public health.
Not access. Not availability. But engagement.
Because adherence doesn’t start with prescriptions.
It starts with conversations.
