Trump just threatened to bomb all of Iran's power plants if Iran doesn't open the Straits within 48h.

We know Iran will not comply with this threat.

If Trump follows through on his threats, Tehran will likely strike at energy facilities throughout the Persian Gulf as well as in Israel. By now, it is really foolish to think it won't.

An already bad situation will worsen with rising oil and gas prices, higher grocery costs in the US, and the closure of most remaining exit ramps.

Escalation only intensifies the lose-lose scenario.

Both sides need to get serious about off-ramps.

Neither side should seek the humiliation of the other. Neither side can force the other's surrender.

Both have to be able to construct a convincing narrative of victory for their own audiences.

They may not realize it yet, but both the US and Iran ultimately have a common interest in such an exit.

There's much speculation about Pezeshkian's video message saying Iran will only strike neighbouring countries that let their territory or airspace be used in attacks against Iran, while also expressing personal regret for the strikes Iran has conducted thus far.

This has fueled speculation that there is a rift between Pezeshkian and the IRGC. While there likely is a rift, this specific message appears more rooted in Pezeshkian's lack of message discipline than a deliberate deviation. The regret he expresses was personal, not on behalf of the state.

It could also be that the message was more specifically intended for Turkey and Azerbaijan, rather than the GCC, since Tehran's calculations differ somewhat when it comes to these neighbours.

But the statement has been met with a lot of anger inside the Iranian system, some of which has been expressed publicly. It has also added pressure to name a new Supreme Leader rather than letting the country be governed by the three-person council (of which Pezeshkian is a member) for much longer.

To date, on the ground, no clear evidence has emerged that Tehran's strategy has shifted.

If anything, Trump's very swift expression of victory after Pezeshkian's statement appears to have reinforced views inside the Iranian system that their strategy - costly as it is - has been working.

Trump simply appeared a bit too eager to declare victory based solely on the video message of a president whom few believe calls the shots in Iran on this war.

"Delayed Treatment is No Treatment and it is Nothing but Torture" (Medical Interpretation and contradictions in the Government's Stance)

The point number 8 in the Salman Safdar reports is terrifying. Compare it with the press conference of PIMS Director Dr Rana Imran Sikandar, it exposes many contradictions in the stance of the government.

1. As per #8, Imran Khan had been complaining of blurred vision and haziness in right eye since Oct 2025 before which his vision was 6/6. His complaint was ignored by the jail authorities and then he suddenly lost complete vision in the right eye which prompted a visit by Dr Arif in jail.

2. We were told by the PIMS director Dr Rana Imran Sikandar that the doctor who visited IK in Adiala jail "performed eyes tests including slit lamp exam, fundoscopy Intra-ocular pressure measurement and ocular tomography of the retina, based on which he made diagnosis of Central Retinal Vein Occlusion (CRVO) and then recommended intra-ocular injection of anti- VEGF for which Imran Khan was taken to PIMS.

3. But IK reports that he was taken to PIMS when he completely lost vision in his right eye (a point not at all mentioned by the PIMS director), and his vision improved only by 15 % to the date after the injection.

4. We were told by PIMS executive director Dr Rana Imran Sikandar that Imran Khan suffered from CRVO (Central Retinal Vein Occlusion) in his right eye. It is very important to note that it is a time sensitive ailment. The prognosis of the disease and success rate of its treatment depends upon the visual acuity at the time of presentation, which depends upon the timing. It implies that a delayed presentation means less visual acuity and more chance of a failed treatment.

As per studies, there are three categories of CRVO when it comes to its prognosis and treatment success:

-Mild: Two-thirds of patients who presented with visual acuity better than 20/40 maintained visual acuity in the same range. (success rate 66 %)

-Moderate: Among patients who presented with visual acuity 20/50 to 20/200, nearly one-half maintained visual acuity in the same range (success rate 50 %).

-Severe: In patients who presented with initial visual acuity of less than 20/200, only 20 percent of patients had some improvement in vision. (success rate only 20 %)

So the success of the treatment and future recovery of the vision depends upon the visual acuity at the time of the presentation.

Now if you read the point 8 of the report again, you will note that the treatment was given only when his vision was completely lost in the right eye. It means his visual acuity must be less than 20/200 (severe category), so the chances of the success of the injection were limited to only 20 %.

How it is Criminal Act:

1. When he was complaining for blurred vision since Oct, he was not offered a complete ophthalmologic assessment that was finally done when he completely lost the vision. Dr Rana Sikandar himself admitted that he was assessed by the ophthalmologist right before he was shifted to PIMS and as per IK, he had already lost complete vision by then. This part was not mentioned by PIMS director at all.

2. If he were offered an earlier assessment, his visual acuity would have been in mild range then that promises 2/3rd (66 %) success rate of the intra-ocular treatment.

3. When he was taken to the PIMS, no medical information was shared with the nation and IK family. No one knows what was his visual acuity at the time of presentation, and what was the prognosis of CRVO in his case?

4. The government kept denying his PIMS visit until it was leaked by journalist @asadullahkhan_. If the sudden loss of vision was being denied by the government, then how can the nation believe that his complaints of blurred vision since October were taken seriously by the jail authorities? There is no mention of an ophthalmologic evaluation from Oct 2025 to Jan 2026 in the pressor of Dr Rana Imran Sikandar. That further confirms these suspicions of the nation that Imran Khan's eye complaints since Oct were trivialized by the authorities.

What should be done now?

The only way to show some humanity and dignity is for the government to form a medical board comprising both government doctors and private ophthalmologists recommended by IK’s doctors, Dr. Faisal Sultan and Dr. Asim Yousaf. They should conduct a complete ophthalmologic evaluation and submit their reports to the Supreme Court.

There is no other way to ensure honesty and transparency. The government has mishandled IK’s case to such an extent that even the most qualified ophthalmologist hired by the government could be pressured into mis documenting findings and would not be considered trustworthy by the nation. Unfortunately, we have stooped to such a level of intellectual dishonesty and corruption. I hope our ophthalmologists will break their silence on this critical issue for the sake of medical oaths they have taken.

Remember: delayed treatment is no treatment, and it falls under the category of torture and neglect.
