#ConceptThatMatters _Why serum amylase level is found to be elevated in perforated peptic ulcer disease? Creating confusion with Acute pancreatitis; as both of them can present with Acute abdomen.
It’s because: rise or elevation of serum amylase is NOT specific for acute pancreatitis or pancreas-related pathologies.
Perforated Peptic Ulcer Disease (PUD) is a well-known cause of high serum amylase.
Majority of peptic ulcers occur in duodenum. Perforation of ulcer is a dreaded complication of Peptic ulcer disease.
When such duodenal ulcers perforate, fluid rich with pancreatic enzymes comes out from perforated duodenum >> subsequently, that gets absorbed by peritoneal lymphatics >> rise in serum level of amylase occurs as a result . Hope this helps !

Systemic findings include low serum protein levels, edema, growth failure, diarrhea, and fatty infiltration of the liver. Name of the disease comes from ‘Ga’ language in Ghana. Interestingly, one medical professional from Ghana confirmed (on my Instagram page) that it truly means so. #pathology360 #DrKishoreBanerjee #MedStudent #MedStudentTwitter
