#ConceptThatMatters #ATaleOf02Cities : a #pathology360 Original : What’s the KEY DIFFERENCE between AML & CML? How to differentiate AML from CML on peripheral blood smear ?Let's make it very simple; I would give you an analogy: IMAGINE there are 02 neighbouring cities : 🔺City-AML & 🔺City-CML. Now in both these cities ,suppose, delivery of newborns have increased: So, in City-AML,newborns are taking birth very fast;but, these newborns are NOT able to mature into toddlers, young adults or adults : their development gets ARRESTED in newborn stage. So, we have too many newborns(Myeloblasts) in the City-AML😀. On the contrary, in CML-city, there is increase in birth and these newborns are maturing normally..so,we have not only increase in newborns (Myeloblasts)but also increased number of toddlers(Myelocytes), pre-schoolers(Metamyelocytes), young adults(band forms) & adults (Mature neutrophils)in the CML-city . 1. If you have understood this analogy given by me well, it should not be difficult to differentiate AML from CML. 2. In AML, we see increased number of cancerous blasts. These blasts in AML divide uncontrollably. But,these blasts CAN NOT differentiate into further mature cells. These precursor blast cells in AML have lost their ability to differentiate. These precursor blood-cells in AML are stuck in “ Blast stage “. It results in progressive increase in blast cells in bone marrow which ultimately spills into peripheral blood. The KEY problem here is that maturation of Myeloblasts mostly gets " ARRESTED " at immature myeloblast stage. These Myeloblasts can not become Myelocyte, Metamyelocytes and ultimately transform into the mature Neutrophils/granulocytes. 3. On the other hand, in CML, there is increased proliferation of Myeloblasts ;but, those Myeloblasts can mature and differentiate upto Mature Neutrophils(granulocytes))(🔺ADULT human being ) 4. So, if you look at bone marrow or peripheral blood smear of AML , you mostly see predominance of blast (Newborn babies 👶 which can not mature). 5. But, in CML, you get mixed populations. Some are babies 👶 (Myeloblasts); but there is also increased number of toddlers(Myelocytes), pre-schoolers(Metamyelocytes), young adults(band forms) & adults (Mature neutrophils) 6.
