🫀The role of ICDs for primary prevention in NICM (non-ischemic cardiomyopathy) remains uncertain.
While early RCTs (DEFINITE, SCD-HeFT) suggested benefit, DANISH showed no mortality reduction—likely due to better HF therapy & fewer arrhythmic deaths.
ICDs remain standard in ICM, but for NICM, decisions should be individualized based on comorbidities, life expectancy, & CRT use.

PA Pulsatility Index = (PASP - PADP) / RAP
A quick invasive marker of RV function!
It reflects the RV’s ability to generate pulsatile flow vs its filling pressure.
So PAPI assesses the RV’s ability to generate pulsatile flow relative to its filling pressure.
📉 Low PAPI suggests RV failure:
<1.85 → RVF after LVAD
<1.0 → RVF in acute MI
