Look at balance of risk, does the presentation, PMH or determinants of health suggest other possible pathology more likely. Consider where you are in the care pathway and safety net.

Makes sense for primary care to do this and hopefully free up rheumatology for more complex work. Primary care is stretched but shorter wait for Rheumatology reduce repeat app in primary care
