-Complexity and Multifactorial Nature: Pulmonary vascular remodeling in PAH is a complex and multifactorial pathophysiological phenomenon.
-Crucial Pathways: Among the molecular mechanisms involved in vascular remodeling, defects in the BMP/TGF-β pathways are particularly crucial in PAH.
-Promising Target: The soluble ACTRIIA IgG-Fc fusion protein sotatercept has shown promise in clinical trials. However, further research is needed to fully understand its mechanisms and optimize its use for reversing PAH-related vascular lesions. #WSPH2024
Professor Guignabert presented on pulmonary vascular remodeling in PH. He explained the process is driven by three interconnected forces:
1. Endothelial Dysfunction- Problems in the cells lining blood vessels.
2. Fibroblast and Smooth Muscle Dysfunction- Issues in the cells responsible for structural support and contraction.
3. Inflammation and Disimmunity- Immune responses that lead to inflammation.
